This paper reports a European Forum for Good Clinical Practice workshop held in 2011 to consider a research ethics committee training syllabus, subsequent training needs and resources. The syllabus that was developed was divided into four competencies: committee working; scientific method; ethical analysis and the regulatory framework. Appropriate training needs for each, with possible resources, were discussed. Lack of funding for training was reported as a major problem but affordable alternatives were debated. Strengths and weaknesses of this approach were discussed (...) and the resultant proposal will be disseminated through the European Forum for Good Clinical Practice and the research ethics committees of member states. In 1952, Quine showed that the problem of reducing a propositional formula to a simplest normal equivalent can be solved in two steps, viz., (i) express the given formula, Φ, equivalently as the disjunction of all its prime implicants, and (ii) find all non-redundant disjunctions of the latter that are equivalent to Φ (Quine 1952). However, it seems not generally known that an ingenious form of the same two-step process was published by Hugh McColl in 1878.

Hugh Mellor has proposed what appears to be a new solution to the problem of intrinsic change (Mellor 1998). Assuming endurantism and a B‐theoretic, nonpresentist view of time, facts are supposed to have only enduring things and atemporal properties (or relations) as constituents, but no times. The having of properties and relations is not relativised to times. Instead, the whole of a fact is conceived of as temporally localised. It will be argued that this interesting and novel proposal does (...) not succeed as an account of change in the intrinsic properties of things. The basic difficulty is that the view still leads into contradiction, since it makes it incomprehensible how one and the same thing can have both a property and some incompatible property. The having of these incompatible properties is treated as two facts. But to add that these facts have certain temporal locations is of no help for avoiding the contradiction. (shrink)

Frank Ramsey was the greatest of the remarkable generation of Cambridge philosophers and logicians which included G. E. Moore, Bertrand Russell, Ludwig Wittgenstein and Maynard Keynes. Before his tragically early death in 1930 at the age of twenty-six, he had done seminal work in mathematics and economics as well as in logic and philosophy. This volume, with a new and extensive introduction by D. H. Mellor, contains all Ramsey's previously published writings on philosophy and the foundations of mathematics. The (...) latter gives the definitive form and defence of the reduction of mathematics to logic undertaken in Russell and Whitehead's Principia Mathematica; the former includes the most profound and original studies of universals, truth, meaning, probability, knowledge, law and causation, all of which are still constantly referred to, and still essential reading for all serious students of these subjects. The Cambridge philosopher Frank Ramsey died tragically young, but had already established himself as one of the most brilliant minds of the twentieth century. Besides groundbreaking work in philosophy, particularly in logic, language, and metaphysics, he created modern decision theory and made substantial contributions to mathematics and economics. The Editors’ Preface to the fourth edition of Wittgenstein's Philosophical Investigations is disparaging of the earlier editorial efforts of G. E. M. Anscombe and Rush Rhees and in particular of their inclusion and titling of the material in “Part II”. I argue, on both historical and philosophical grounds, that the Editors have failed to refute the editorial decisions of Rhees and Anscombe – a failure born both of a neglect of the historical circumstances and Wittgenstein's own expressed hopes and intentions (...) for his writings, and of a myopic understanding of his philosophy. Wittgenstein's legacy has not been well served by their interventions, which should be undone in future editions. (shrink)

The problem of knowledge is generally regarded as an essential part of philosophy, and since the time of Descartes at least, every philosopher has found it necessary to show that his own view can afford a solution to the problem. In this paper, however, the method of approach is psychological rather than philosophical, and an attempt is made to deal with the question from a point of view differing in several respects from those more commonly accepted.

Wittgenstein's scant remarks on the roots of language in instinctive behaviour have been both difficult to interpret and controversial, not least because they may seem to incline towards forms of explanation that elsewhere he eschewed. Nevertheless, they are of importance in philosophy, not least because they bear upon age-old questions of foundationalism and concept-formation. In a recent Discussion Note in this journal, H. O. Mounce is not only attracted by but also champions such explanation – though he finds Wittgenstein's own (...) “explanations” inadequate in important respects, as evidenced in his misguided criticisms of Moore in On Certainty and in his behaviouristic remarks on the roots of the “cause and effect” language-game. In this response, I try to identify the philosophical roots of this attraction and the points at which Mounce's account departs from the elucidatory and non-explanatory path established by Wittgenstein. Rush Rhees identified the question of “what language is” as central to Wittgenstein's philosophy, but believed he failed to follow up adequately the connections between the reality of discourse and our reality as persons. Integral to this is Rhees's elaboration of the distinction between such investigations into language and approaches to philosophy restricted to elucidating “the grammars of particular expressions.” The failure to fully acknowledge Rhees's contribution to the understanding of these issues has vitiated recent New Wittgensteinian discussion of both (...) A critical discussion is provided of three central assumptions underlying Nunez's approach to modeling cortical activity. A plea is made for neurophysiologically realistic models involving nonlinearities, multiple time scales, and stochasticity.

Mild and moderate mental illnesses can hinder labor force participation, lead to work interruptions, and hamper earning potential. Targeted interventions have proven effective at addressing these problems. But their potential depends on labor protections that enable people to take advantage of these interventions while keeping jobs and income.

In the first part of this article an attempt was made to clear the ground for a functional theory of knowledge, and the discussion of structure and function with which it concluded enables us to approach the problem of cognition. If the view already set forth is sound, it seems clear that the relation of the mind to its object is a function and not a structure of the mental processes involved. The mere existence of a mental content, however complex (...) it may be, however decorated with fringes or set off by a background, does not constitute an act of knowledge or give the content a meaning. In order to see a mental act as cognitive we must consider its function, its origin, its results, and in general the circumstances in which it stands. (shrink)

Quantum theory of Lorentz invariant local scalar fields without restrictions on 4-momentum spectrum is considered. The mass spectrum may be both discrete and continues and the square of mass as well as the energy may be positive or negative. One may assume the existence of such fields only if they interact with ordinary fields very weakly. Generalization of Kallen-Lehmann representation for propagators of these fields is found. The considered generalized fields may violate CPT-invariance. Restrictions on mass-spectrum of CPT-violating fields are (...) found. Local fields that annihilate vacuum state and violate CPT-invariance are constructed in this scope. Correct local relativistic generalization of Lindblad equation for density matrix is written for such fields. This generalization is particularly needed to describe the evolution of quantum system and measurement process in a unique way. Difficulties arising when the field annihilating the vacuum interacts with ordinary fields are discussed. (shrink)

The Metaphysical Club, a gathering of intellectuals in the 1870s associated with Harvard, is widely recognized as the crucible where pragmatism, America's distinctively original philosophy, was refined and proclaimed. Louis Menand's bestseller about the group was a dramatic publishing success. However, only three actual members - Oliver Wendell Holmes Jr., Charles S. Peirce, and William James - appear in this book, alongside other thinkers such as John Dewey who were never in the Club. The Real Metaphysical Club tells the full (...) story of how this influential group shifted the course of philosophy in America. In addition to pioneering pragmatism, the group explored radical empiricism and idealism, and formulated personalism and process philosophy, equally important developments. This volume contains thirty-seven important writings dating from 1870 to 1885 by the real members of the Metaphysical Club. The first section centers on pragmatism and science; the second part collects writings of the lawyers; and the third part covers idealist and personalist philosophers. Many of these writings have never been reprinted before, and nothing like this impressive collection has ever been attempted. A general introduction provides a narrative history, and the editors' three introductions to the volume's sections vividly bring to life the intense meetings, sustained debates, and pioneering thought from the Metaphysical Club. This book takes up the central themes of Aristotle's metaphysical theory and the various transformations they undergo prior to their full expression in the Metaphysics. Aristotle's metaphysics is bedevilled by classic puzzles involving such notions as form, predication, universal, and substance, which result from his attempt to adapt the various requirements on primary substance developed in his earlier works so that they fit the very different metaphysical picture in his later work. Professor Lewis argues that Aristotle is himself aware of (...) most if not all of these difficulties and in the Metaphysics works hard to ensure the coherence of his theory. An attempt to bring some of the major issues and debates in the philosophy of social research up-to-date. There is a new chapter on the philosophy of science, the conclusion has been rewritten and other chapters have been updated.

Compared to our understanding of positive prediction error signals occurring due to unexpected reward outcomes, less is known about the neural circuitry in humans that drives negative prediction errors during omission of expected rewards. While classical learning theories such as Rescorla–Wagner or temporal difference learning suggest that both types of prediction errors result from a simple subtraction, there has been recent evidence suggesting that different brain regions provide input to dopamine neurons which contributes to specific components of this prediction error (...) computation. Here, we focus on the brain regions responding to negative prediction error signals, which has been well-established in animal studies to involve a distinct pathway through the lateral habenula. We examine the activity of this pathway in humans, using a conditioned inhibition paradigm with high-resolution functional MRI. First, participants learned to associate a sensory stimulus with reward delivery. Then, reward delivery was omitted whenever this stimulus was presented simultaneously with a different sensory stimulus, the conditioned inhibitor. Both reward presentation and the reward-predictive cue activated midbrain dopamine regions, insula and orbitofrontal cortex. While we found significant activity at an uncorrected threshold for the CI in the habenula, consistent with our predictions, it did not survive correction for multiple comparisons and awaits further replication. Additionally, the pallidum and putamen regions of the basal ganglia showed modulations of activity for the inhibitor that did not survive the corrected threshold. (shrink)

Aim: Patients with advanced cancer need information about end-of-life treatment options in order to make informed decisions. Clinicians vary in the frequency with which they initiate these discussions.Patients and methods: As part of a long-term longitudinal study, patients with an expected 2-year survival of less than 50% who had advanced gastrointestinal or lung cancer or amyotrophic lateral sclerosis were interviewed. Each patient’s medical record was reviewed at enrollment and at 3 months for evidence of the discussion of patient wishes concerning (...) ventilator support, artificial nutrition and hydration , resuscitation and hospice care. A Kaplan–Meier analysis was also performed and 2-year survival calculated.Results: 60 cancer and 32 ALS patients were enrolled. ALS patients were more likely than cancer patients to have evidence of discussion about their wishes for ventilator support , ANH , DNR and hospice care . At 6 months, 91% of ALS patients were alive compared with 62% of cancer patients; at 2 years, 63% of ALS patients were alive compared with 23% of cancer patients .Conclusions: Cancer patients were less likely than ALS patients to have had documented advanced care planning discussions despite worse survival. This may reflect perceptions that ALS has a more predictable course, that advanced cancer has a greater number of treatment options, or differing views about hope. Nevertheless, cancer patients may be less adequately prepared for end-of-life decision-making. (shrink)
